  • National Pathology and Diagnostics November 2020 Newsletter

    Professor Mike Richard's 2020 review of diagnostics capacity

    As many of you may be aware, Professor sir Mike Richards was recently commissioned by NHS Chief Executive Sir Simon Stevens to review diagnostic services as part of the NHS Long Term Plan. The review's proposals will help save lives and improve people’s quality of life including for cancer, stroke, heart disease and respiratory conditions.

     Within his review, Sir Mike recommended that NHS Digital’s work on developing and implementing a standardised universal test list across all diagnostic disciplines (pathology, imaging, endoscopy and cardiorespiratory services) should be accelerated.

    Our work with the Professional Records Standards Body (PRSB)

    The Pathology business product team have continued to support the PRSB in their work which aims to  show how the strategic standards we have developed (SNOMED based unified test list (UTL), Unified Code for Units of Measure (ucum) for units of measure (UoM) and FHIR messaging to transmit UTL and UoM) are feasible to implement using demonstrators, and to gather feedback from stakeholders through engagement on the feasibility of their implementation

    Metadataworks have been involved in the first phase of this work and have been  developing mappings from the read code based Pathology bound code list (PBCL)  to the UTL  for commonly used Pathology results, assuring the mappings with Pathology experts and utilising an application programming interface (API)for system to query mappings

    The Pathology Business Product team have been analysing some of the outputs of PRSB's work on the Unified test list including the UTL-PBCL list comparison and the unreleased UTL content PBCL match analysis, and have been producing new Microbiology content for the forthcoming release of the UTL

    UTL release schedule

    Following the recent announcement of changes to the SNOMED CT UK combined release and the feedback received for Pathology Beta Human readable releases (v0.4.0 and v 0.5.0) we have revised the Pathology Unified Test List (UTL) Release schedule as follows.

    • The Pathology Laboratory Medicine SNOMED CT content will not be part of the revised November combined release but will be aligned for the next, 1 April 2021 UK Edition Release
    • UTL V0.5.0 will be revised following feedback received from stakeholders following the Beta release in July and verified by the Microbiology SME sub-group and the Pathology Standards Governance Board

    In particular, on the 20th October during the Microbiology SME sub-group it was determined that the defined microbiology presence pattern was inconsistent with current user ways of working and will be revised for UTL v0.6.0 to incorporate the feedback received into a preferred pattern that better suits user needs and determine a preferred option for subsequent release.

    • 2 further UTL Human Readable releases are scheduled for January and March 2021 to complete the Blood Sciences Reporting requirements and, align the Pathology Bounded Code list and recent COVID tactical solution with the strategic UTL Model

    Thank you all for your valuable feedback, we will continue to engage with interested stakeholders via the Clinical Alpha reviews, UTL speciality sub groups, Pathology Informatics National User Group (PINUG) and obtain assurance from the Pathology Standards Governance Board to ensure that user needs are met before full SNOMED CT Release alignment.

  • Updated guidance for the national naming, coding and processing of SARS-COV-2 Pathology tests

    NHS Digital have recently updated the guidance for the national naming, coding and processing of SARS-COV-2 Pathology tests which now contains additional information for Labs and LIMS on naming requestable and reportable items related  to SARS-COV-2 Testing.

    Changes in v2.5.9:

    • Updated ConceptIDs and DescriptionIDs for new RNA Positive, and RNA Negative SNOMED CT concepts included in the UK SNOMED CT November 2020 Release

    • Added Lateral Flow Assay for Antigen testing

    Changes in v2.5.8:

    • Added “RNA Positive at the Point of Detection” concept and associated elements

    Changes in v2.5.7:

    • Updated section 4.3 to remove FSN DescriptionIDs for some rows

    • Additional information on the SNOMED CT mapping refset files

    Changes in v2.5.4:

    • Working with the RCPath and the PSGB we have created a detailed set of requestable and reportable items to help laboratories and LIMS suppliers define COVID19 related data items. These include:
      • Requestable Items for Virus detection (including Antigen) and Antibody detection
      • Reportable Items for each detection technique
      • Permissible values for each result
    • Updated guidance on mapping from lab results to SNOMED CT via a set of ‘standardised descriptions’ used to flow the data in the PMIP NHS003 Message
    • Updates to SNOMED CT findings to allow filing of results into Primary Care EPR systems
    • Updates to the structure of the machine readable mapping table now included in the SNOMED CT UK Clinical Extension as a pair of simple map refsets

  • National Pathology and Diagnostics September 2020 Newsletter

    UTL release schedule

    Dear all,

    In our previous newsletter we notified you of the publication of the latest iteration of the Unified test list v0.5.0 in human readable form on our Pathology and Diagnostics Kahootz site.

    The 139 new concepts in this 1st July release included a sample of microorganism nucleic acid SNOMED CT reporting concepts by presence of RNA/DNA, and a  sample of microorganism susceptibility to antimicrobial SNOMED CT reporting concepts.

    We would like to thank all those who attended the last PINUG meeting and subsequent UTL feedback workshop where we presented an overview of the UTL v0.5.0 release, received critical SME feedback on the release content and solicited interest from LIMS suppliers and providers in setting up focused sub groups to address questions relating our Pathology information standards in development (UTL, Units of measure and FHIR message specification).

    For those subscribed to TRUD you will have recently received a notification regarding the revised autumn 2020 schedule for UK SNOMED releases. As previously communicated, we have made the decision to align the release schedule of the UTL PaLM extension module on TRUD with the UK edition release schedule.

    The next UTL release (v0.6.0) will therefore coincide with the next major UK edition release, scheduled for the 4th November.

    Pathology Informatics National User group (PINUG)

    The Pathology Informatics National User Group (PINUG) aims to encourage a two-way dialogue between NHS Digital and other interested parties, allowing us to successfully align with all stakeholder group requirements.   

    The group meets every two months via MS Teams and consists of clinicians, diagnostic professionals and system suppliers.  The meetings are chaired by our lead clinician, who is a member of the Royal College of Pathologists (RCPath).

    The next PINUG is scheduled to take place on 8th September and will involve a discussion around the UTL v0.5.0 release feedback received at the UTL workshop in July, further information relating to the Proposed PINUG subgroups and recently updated education and training materials

    Our work with the Professional Records Standards Body (PRSB)

    The Pathology business product team have recently undertaken a procurement exercise for the provision of Terminology skills and subject matter expertise to support strategic Pathology activities and are pleased to announce that the Professional Records Standards Body (PRSB) have been commissioned for this work.

    The PRSB will build and establish artefacts to showcase the feasibility of using our Pathology information standards in live systems, and will identify key stakeholders for 'first of type' demonstrators fit for submission to the Data coordination Board as evidence for replacement of the PBCL and replacement for EDIFACT Pathology messaging.
