The SNOMED CT UK Drug extension is not released as part of the SNOMED CT biannual release. SNOMED CT UK Drug Extension data is published on a four-weekly cycle within the "UK Drug Extension, RF2" sub pack on TRUD. Please review the associated documentation for more details.

Release documentation is now only accessible on DELEN, at https://hscic.kahootz.com/t_c_home/view?objectId=16607376

Please see the ‘Future release schedule’ section in this notice for important information about the release timetable and content for the remainder of the 2020 calendar year. Key points:

    • Larger August / October releases, to bring the UK Edition into alignment with the International Edition.
    • The possibility of moving to a more frequent release schedule in 2021.
    • Changes to the SNOMED CT UK Edition release subfolder structure.

Important changes and information regarding this release:

This is the fifth emergency COVID-19 “microrelease”. Changes are restricted to coding expressivity improvements urgently needed to manage the current pandemic. This very narrowed scope should reduce the “content update penalty” on product consumers, and so reduce barriers to the urgent deployment of the new content to the clinical frontline.

This release remains based on the July 2018 International Edition.

A new tranche of 11 COVID-19 codes are added relating to the current pandemic, in addition to those already released for that same purpose in the 28.1.1, 29.0.0 and 29.1.0 Emergency Releases of February and April 2020. The new codes are:

1322781000000102       Severe acute respiratory syndrome coronavirus 2 antigen detection result positive (finding)

1322791000000100       Severe acute respiratory syndrome coronavirus 2 antigen detection result negative (finding)

1322801000000101       Severe acute respiratory syndrome coronavirus 2 antigen detection result indeterminate (finding)

1322821000000105       Severe acute respiratory syndrome coronavirus 2 antigen detection result unknown (finding)

1322831000000107       Severe acute respiratory syndrome coronavirus 2 immunoglobulin G detection result indeterminate (finding)

1322841000000103       Severe acute respiratory syndrome coronavirus 2 immunoglobulin A detection result indeterminate (finding)

1322851000000100       Severe acute respiratory syndrome coronavirus 2 immunoglobulin M detection result indeterminate (finding)

1322871000000109       Severe acute respiratory syndrome coronavirus 2 antibody detection result positive (finding)

1322891000000108       Severe acute respiratory syndrome coronavirus 2 antibody detection result negative (finding)

1322901000000109       Severe acute respiratory syndrome coronavirus 2 antibody detection result indeterminate (finding)

1322911000000106       Severe acute respiratory syndrome coronavirus 2 antibody detection result unknown (finding)

In addition to the new COVID-19 concept codes above, the Preferred Term has been changed for the following already released non-COVID-19 concept code:

    1035881000000100

Commission for Health Improvement Experience of Service Questionnaire self-report for 12-18 year olds item 3 score - I was treated well by the people who saw me (observable entity)

    PT

    Future release schedule

Following a formal consultation with stakeholders, a schedule of releases for the remainder of the current calendar year is now published, including two much larger releases of non-COVID-19 content on 12 August and 7 October.  Between them, these two major releases will enable the publication of all newer versions of SNOMED International content released since July 2018, plus UK content deferred from the original 1 April release candidate, and some of the backlog of other new UK content requests unrelated to COVID-19. The Results of the formal consultation on the SNOMED CT UK Edition Release schedule are available on Delen.

The schedule for these major releases through to the end of 2020 will be as follows:

    12 August

    TRUD Version  30.0.0 effectiveTime    20200805

Base content:   July 2019 International Edition

New content:    All UK content deferred from the 1 April release

Some additional non-COVID-19 content for the UK                         Any requested new COVID-19 content (as per a microrelease)                         Partial ICD10 and OPCS maps covering above new content                         “Light touch” update of refsets to align with new content

    7 October

    TRUD Version  31.0.0 effectiveTime    20200930

Base content:   July 2020 International Edition

New content:    Some additional non-COVID-19 content for the UK                         Any requested new COVID-19 content (as per a microrelease)                         Full ICD10 and OPCS maps covering above new content                         Full update of refsets to align with new content

    2 December

    TRUD Version  31.2.0 effectiveTime    20201125

Base content:   July 2020 International Edition

New content:    Some additional non-COVID-19 content for the UK                         Any requested new COVID-19 content (as per a microrelease)                         ICD10 and OPCS map updates covering above new content                         Full update of refsets to align with new content

NB the opportunity and need for further emergency unscheduled microreleases of COVID-19 specific content in addition to the above schedule remains a possibility, although they would always coincide with an already scheduled 4-weekly release of the UK SNOMED CT Drug Extension. Should this happen, the only possible dates for additional microreleases are therefore 15 July, 9 September, 4 November, and 30 December, but currently there are no plans for any such microreleases.

The subsequent timetable of scheduled releases into 2021 and beyond is not yet finalised – a further consultation on this will occur later in Autumn 2020. Our current preferred option is to continue indefinitely with scheduled releases occurring every 8 weeks, with ad hoc microreleases also able to occur at the 4-weekly mid-point in between those 8-weekly scheduled releases, but with all releases always coinciding with a 4-weekly release of the UK drug extension.

Planned changes to the SNOMED CT UK Edition release subfolder structure

Subscribers were previously advised that all future PRODUCTION releases of the Drug and Clinical extension content would transition from April 2020 to the BETA release bundling previously provided as technology previews (Drug and Clinical extension content).

For a more detailed explanation of the motivations for the BETA bundling, see the webinar recording at https://hscic.kahootz.com/t_c_home/view?objectid=54390437. Detailed information is also provided in the UK Clinical Edition SNOMED CT Technical overview document, section 2.4.1 and 2.4.2.

To reduce burden during the pandemic, this transition was deferred. Suppliers remain recommended to transition to consuming the BETA rather than PRODUCTION bundlings, in accordance with the following revised schedule:

Throughout the remainder of 2020, PRODUCTION releases will remain as before but with the same content always co-published on TRUD in BETA format, as Test Releases. Clinical Edition and Drug Extension 29.3.0, RF2, BETA: Full, Snapshot & Delta

From the first scheduled release in 2021 (27 January), all PRODUCTION releases of UK Clinical and Drug will adopt the BETA encoding. All releases will then continue to be available in parallel in the older LEGACY encoding until the end of March 2021, after which the LEGACY encoding will be permanently withdrawn.

    Reference sets (Refsets)

Two new UK mapping sRefsets are added to the 17 June 2020 release:

1323081000000108       Coronavirus disease 19 caused by severe acute respiratory syndrome coronavirus 2 test result communication to general practice concept simple map reference set (foundation metadata concept)

1323091000000105       Coronavirus disease 19 caused by severe acute respiratory syndrome coronavirus 2 test result communication to general practice description simple map reference set (foundation metadata concept)

These hold the equivalences between COVID-19 Laboratory Result “short codes” (e.g. SARS-COV-2-ORGY) and their corresponding SNOMED conceptIds and descriptionIds.

    Cross Mapping

The SNOMED CT to OPCS-4.8, OPCS-4.9 and ICD-10 5th Edition five character maps are included in this 17 June 2020 bi-annual release; there are no changes in this release to either map.

  • Guidance for naming, coding and processing SARS-COV-2 Pathology Tests

NHS Digital have recently updated the guidance for the national naming, coding and processing of SARS-COV-2 Pathology tests which now contains additional information for Labs and LIMS on naming requestable and reportable items related  to SARS-COV-2 Testing.

    Changes in v2.5.4:

    • Working with the RCPath and the PSGB we have created a detailed set of requestable and reportable items to help laboratories and LIMS suppliers define COVID19 related data items. These include:
      • Requestable Items for Virus detection (including Antigen) and Antibody detection
      • Reportable Items for each detection technique
      • Permissible values for each result
    • Updated guidance on mapping from lab results to SNOMED CT via a set of ‘standardised descriptions’ used to flow the data in the PMIP NHS003 Message
    • Updates to SNOMED CT findings to allow filing of results into Primary Care EPR systems
    • Updates to the structure of the machine readable mapping table now included in the SNOMED CT UK Clinical Extension as a pair of simple map refsets

This document describes the mapping table changes in the latest version of the guidance
