The Professional Records Standards are a key requirement for designing forms and documents which are understandable across the system. They need to be a higher priority for Mental Health Trusts. Data Standards are a good start. However a great deal of work is needed to understand how and when information is captured, whether the information is valid and whether its capture causes burden or inhibits the standardisation agenda. The text describing ICD mentions only the use of the classification in England. The international face of ICD should also be mentioned (as it is with SNOMED) i.e. "ICD is the foundation for identification of health trends and statistics globally, and the international standard for reporting diseases and health conditions. Snomed CT and ICD11 are both referred to in the future of healthcare and the framework. However the two seem to have been merged in some way re their purpose e.g. ICD11 derived from operational narrative in Snomed CT, this suggests the operational and research usecases are not fully understood particularly retrospective review of events represented by professional coding

It would have been worth mentioning here that ICD is used in more than 100 countries to report mortality data. ICD-10 is also used within the Commissioning Data Sets (along with OPCS-4) to derive HRGs, which are units of currency used to reimburse Trusts under the National Tariff Payment System.
