  • Clinical Coding Auditor & Trainer Programme Frameworks Service Improvement consultation

    Dear colleague,

    We have issued the following Clinical Coding Auditor & Trainer Programme Frameworks Service Improvement consultation which describes how elements of the Clinical Coding Auditor Programme (CCAP) and Clinical Coding Trainer Programme (CCTP) frameworks can be adapted/expanded to make the requirements frameworks more robust. Many of the suggestions have already been discussed with, and in some cases suggested by, existing approved clinical coding auditors and trainers.

    We would appreciate your comments on the proposal by Tuesday 30th June 2020.

  • National Interim Clinical Imaging Procedure (NICIP) Code Set - April 2020 release information

    Dear Colleague,

    The biannual release of new National Interim Clinical Imaging Procedure (NICIP) code set content scheduled for 1 April 2020 will only include the NICIP-OPCS-4.9 mapping update and no changes to NICIP-SNOMED CT mappings.  

    All other NICIP files will be static from the June 2019 release and can continue to be used though files dated with the April 2020 release are included but remain static.  The next full release will again follow our schedule for October 2020. 

    The content originally planned to be released on 1 April 2020 was expected to include NICIP to SNOMED CT map updates to new international SNOMED CT content. In light of the current pandemic, we recognise that large scale changes to the coding framework could impose significant assurance and change management burden on suppliers, which may delay the deployment of care-critical SNOMED CT COVID-19 content.  Because of this a limited NICIP release as described above will be available.

  • COVID-19 and ICD-10 Classification

    Dear Colleague,

    A dedicated COVID-19 area has been launched on Delen to provide: COVID-19 information from the World Health Organisation; information on the classification of COVID-19 in ICD-10; and a single authoritative location for COVID-19 national clinical coding standards for Admitted Patient Care in England. This area will be updated as more information becomes available so please check back regularly for updates.

  • UK SNOMED CT Clinical Edition – April 2020 release – COVID-19 content

    Dear colleague,

    The planned biannual release of the UK SNOMED CT Clinical Edition on the 1st April will be replaced with a significantly smaller version in order to add COVID-19 content and a small amount of high priority concepts.

    The content originally planned to be released on the 1 April 2020 was expected to include additions to the following domains digital child health, new-born blood spot screening, IAPT and mental health as well as new international content.   However, in light of the current pandemic, we recognise that such large-scale changes to the coding framework could impose a significant assurance and change management burden on system suppliers, which may delay the deployment of any care-critical COVID-19 elements. Because of this the full release with these additions has been cancelled. 

    Further similarly small COVID-19 releases may follow in the weeks ahead as new coding requirements emerge.  Any other interim requirements will be considered in consultation with the wider NHS and relevant stakeholders.

    The next full release will follow our schedule for October 2020.

  • NHS Digital Classifications Browser – OPCS-4.9 Now Available

  • Chemotherapy Regimens Clinical Coding Standards and Guidance

    Dear colleague,

    The Chemotherapy Regimens Clinical Coding Standards and Guidance April 2017 has been updated, for use from 1 April 2020, to remove any reference to the use of high cost drugs to align with the changes made to the High Cost Drugs Clinical Coding Standards. The chemotherapy Procurement and Delivery codes (X70-X74) have not changed in OPCS-4.9 and the regimens list 17-18 remains the same. Coders should continue to use the 17-18 regimens list from April 2020.

    We have previously communicated that the April 2017 standards and regimens would the last planned update to the regimen list by NHS England and NHS Improvement in this format as they are currently working on a revised approach to capturing information about chemotherapy delivery mechanisms and generating appropriate reimbursement over the next eighteen months ready for implementation in 2019/20. This revised approach is still being agreed and the collection of OPCS-4 codes for chemotherapy are still required.

    We will communicate further when the revised approach has been agreed and the effect on OPCS-4 codes and standards is known.

    The Chemotherapy Regimens Clinical Coding Standards and Guidance 2020 and the Regimens List 17-18 are available from Delen and will be accessible in the Classifications Browser shortly.

  • 2020-21 Classifications Training Schedule

    Dear Colleague,

    Following the national NHS and government guidance on the outbreak of the coronavirus (COVID-19) we have reviewed our training delivery schedule to ensure that we align with national guidance and operate in the best interests of the wider community.  This has led to a decision to postpone the following events/courses on our 2020/21 Training Schedule:

    • Trainer Refresher Course (TR1) in London on 12-14 May 2020 and any associated Re-Certification assessments
    • CCAP Assessment Day in Leeds on 20 May 2020
    • CCTP Assessment Day in Leeds on 10 June 2020

    We realise that this will be a huge disappointment but hope you agree it is important to adhere to the national recommendations to reduce the spread of coronavirus.  As more information emerges we may need to consider postponing other courses or reschedule but will monitor the situation and publish updates accordingly.

    Candidates already accepted on to the CCAP or CCTP Assessment Days will not have to re-apply as your applications and Purchase Order / other payment details will automatically be carried across to any rescheduled date. Similarly those trainers who have booked on to the May Trainer Refresher Course will not have to send in a further booking form or payment information as these will automatically carry across to any rescheduled date.

    Some approved clinical coding trainers are due or overdue their Trainer Refresher Course and/or their Re-Certification assessment which will be affected by this decision.  Some approved trainers may have to cancel national training courses as a result of the coronavirus guidance, self-isolation or social distancing.  This could impact on your ability to meet the 21 day mandatory training delivery requirement during 2020/21. As this situation is beyond our control, no-one will have their ‘approved’ clinical coding trainer status removed due to either of these circumstances.

  • OPCS-4.9 implementation on 1 April 2020

    Dear colleague,

    The updates to the OPCS-4 classification to produce OPCS-4.9 were released in November 2019 and will continue to the usual schedule with NHS and system supplier implementation on 1 April 2020.

    The OPCS-4.9 ISN mandate was issued in October 2019 for the NHS and system suppliers and the OPCS-4.9 data files were released on the NHS Digital distribution platform (TRUD) in November 2019.

    On 2 March 2020 NHS Digital published on TRUD patch release of the SNOMED CT to OPCS-4.9 map with associated guidance to support suppliers with SNOMED CT enabled systems.

    The HRG4+ 2020/21 Local Payment Grouper and the 2020/21 Prescribed Specialised Services Operational Tool will be released w/c 23 March 2020, for use from 1 April 2020, available via the landing page at https://digital.nhs.uk/services/national-casemix-office/downloads-groupers-and-tools .  These include updates for the OPCS-4.9 release, to support NHS reimbursement and commissioning. Updates have also been included in the Grouper to accommodate changes in diagnosis coding in relation to COVID-19.

    If Trusts find that suppliers are unable to implement the OPCS-4 updates please advise the classifications service with relevant information via information.standards@nhs.net so we can engage directly with the supplier to discuss the issues and how we can help to support supplier compliance.

  • Coding of COVID-19

    Published: March 2020

    Dear Colleague,

    We have received a number of queries seeking confirmation of ICD-10 coding of suspected cases of COVID-19 and the application of the national clinical coding standard DCS.XXII.5: COVID-19 (U07.1). We are currently working closely with WHO through the WHO-FIC Classifications and Statistical Advisory Committee on additional coding guidance for COVID-19. 

    Please be assured this is being given the highest priority and once the guidance has been agreed we will publish the resolutions on the Query Resolution Database for the benefit of the wider coding community.
