  • OPCS-4 Chemotherapy Procurement Codes

    The OPCS-4 National Clinical Coding Standard PCSX27: Delivery of chemotherapy for neoplasm (X72-X73) states that:

    The collection of codes classifying procurement is not required, therefore codes from categories X70 and X71 should not be assigned.

    Note that this statement does not prevent the use of procurement codes where trusts wish to do so for local purposes.

    This will be clarified in an updated release of the OPCS-4 National Clinical Coding Standards reference book which will be published shortly.

  • Update to COVID-19 National Clinical Coding Standards and Guidance – nMABs

    Published November 2021

    Dear Colleague, 

    The COVID-19 National Clinical Coding Standards have been updated to include a coding standard for Neutralising monoclonal antibodies (nMABs) for the treatment of COVID-19 for episodes ending on or after 01 November 2021.

    The downloadable format of the COVID-19 National Clinical Coding Standards (01 November 2021) has been updated along with the summary of changes.

  • National Clinical Coding Standards ICD-10 and OPCS-4 Reference Books (2021)

    Dear colleague,

    The National Clinical Coding Standards ICD-10 and OPCS-4 reference books for use from 1 April 2021 are available for download from the Publications & Resources page on Delen.

    The OPCS-4 and ICD-10 Reference Books will be available in the Classifications Browser and will be made available on TRUD for addition into the eVersion shortly. Further communications will be issued when they are available.

    Please note the following:

    • In the OPCS-4 reference book there is reference to Coding Clinic Ref 121: Procedures performed for female pelvic organ prolapse and stress urinary incontinence which will contain a guidance table with procedures performed for female Stress Urinary Incontinence and Pelvic Organ Prolapse that are not easily reached using the OPCS-4 Alphabetical Index. This guidance is being finalised and will be published in the next issue of the Coding Clinic.
    • The COVID-19 National Clinical Coding Standards and Guidance are referenced within the ICD-10 Reference Book but will remain on Delen for the time being so that we can continue to quickly update the standards to reflect developing clinical knowledge of COVID-19, problems identified from helpdesk resolutions and any changes provided to us by WHO, ensuring you have the most up to date information to aid accurate and timely coding. However, we will be providing the COVID-19 Standards and Guidance in separate downloadable format.

  • April 2020 SNOMED CT Biannual Release (includes COVID-19 content)

    The SNOMED CT UK Clinical Edition, RF2 for April 2020 is now available for download from the following links:

    The SNOMED CT UK Drug extension is not released as part of the SNOMED CT biannual release. SNOMED CT UK Drug Extension data is published on a four-weekly cycle within the "UK Drug Extension, RF2" sub pack on TRUD. Please review the associated documentation for more details.

    Release documentation is now only accessible on Delen.

    Important changes and information regarding this release

    Although a release was always scheduled for April 2020, its content changes have been radically descoped and are now restricted to coding expressivity improvements urgently needed to manage the current pandemic. This very narrowed scope should reduce the “content update penalty” on product consumers, and so reduce barriers to the urgent deployment of the new content to the clinical frontline:

    Whereas this release had been planned to be based on the July 2019 International Edition, it remains instead based on the July 2018 International Edition.

    A new tranche of 30 COVID-19 codes are added relating to the current pandemic, in addition to those already released for that same purpose in the 28.1.1 Emergency Release of February 2020. The 30 new codes are:

    Further incremental “microreleases” of the UK SNOMED CT Clinical Extension will occur with new COVID-19 related content for the NHS. These will coincide with at least the next three scheduled releases of the UK SNOMED CT Drug Extension (22 April, 20 May and 17 June). A full release comprised of substantially all new content deferred from this current scheduled 1 April release is planned for October 2020. However, the ongoing effects and requirements of the pandemic may require this plan to change; the final contents of the October 2020 release will be informed by a consultation that NHS Digital will carry out in July to assess readiness of the NHS and its IT suppliers.

    Subscribers were previously advised that, from April 2020 all future PRODUCTION releases of the Drug and Clinical extension content would transition to the BETA release bundling previously provided as technology previews here. This transition is also now deferred: the PRODUCTIION release will remain as before; BETA format releases will continue to be published alongside.

    Reference sets (Refsets)

    There are 4 new UK refsets for the April 2020 release, as detailed above. All are relevant to COVID-19.

    Cross Mapping

    The SNOMED CT to OPCS-4.8, OPCS-4.9 and ICD-10 5th Edition five character maps are included in this April 2020 bi-annual release. The ICD-10 four character only maps reference set (999002261000000108) has been removed completely from the release from June 2019 onwards.

  • UK SNOMED CT Clinical Edition – April 2020 release – COVID-19 content

    Dear colleague,

    The planned biannual release of the UK SNOMED CT Clinical Edition on the 1st April will be replaced with a significantly smaller version in order to add COVID-19 content and a small amount of high priority concepts.

    The content originally planned to be released on the 1 April 2020 was expected to include additions to the following domains digital child health, new-born blood spot screening, IAPT and mental health as well as new international content.   However, in light of the current pandemic, we recognise that such large-scale changes to the coding framework could impose a significant assurance and change management burden on system suppliers, which may delay the deployment of any care-critical COVID-19 elements. Because of this the full release with these additions has been cancelled. 

    Further similarly small COVID-19 releases may follow in the weeks ahead as new coding requirements emerge.  Any other interim requirements will be considered in consultation with the wider NHS and relevant stakeholders.

    The next full release will follow our schedule for October 2020.

  • OPCS-4.9 implementation on 1 April 2020

    Dear colleague,

    The updates to the OPCS-4 classification to produce OPCS-4.9 were released in November 2019 and will continue to the usual schedule with NHS and system supplier implementation on 1 April 2020.

    The OPCS-4.9 ISN mandate was issued in October 2019 for the NHS and system suppliers and the OPCS-4.9 data files were released on the NHS Digital distribution platform (TRUD) in November 2019.

    On 2 March 2020 NHS Digital published on TRUD patch release of the SNOMED CT to OPCS-4.9 map with associated guidance to support suppliers with SNOMED CT enabled systems.

    The HRG4+ 2020/21 Local Payment Grouper and the 2020/21 Prescribed Specialised Services Operational Tool will be released w/c 23 March 2020, for use from 1 April 2020, available via the landing page at https://digital.nhs.uk/services/national-casemix-office/downloads-groupers-and-tools .  These include updates for the OPCS-4.9 release, to support NHS reimbursement and commissioning. Updates have also been included in the Grouper to accommodate changes in diagnosis coding in relation to COVID-19.

    If Trusts find that suppliers are unable to implement the OPCS-4 updates please advise the classifications service with relevant information via information.standards@nhs.net so we can engage directly with the supplier to discuss the issues and how we can help to support supplier compliance.

  • SNOMED CT to OPCS-4.9 Mapping File Patch Release

    Dear colleague,

    Background

    The SNOMED CT UK Edition files which include the SNOMED CT to OPCS-4.9 mapping files will be published on 1 April 2020, which is the same date that OPCS-4.9 is implemented for use in consultant episodes which finish on or after 1 April 2020.

    In previous revisions of OPCS-4 (for example 4.8 in 2017, 4.7 in 2014), the SNOMED CT UK Edition release (including the mapping files) have been available as an early release by the second week in March. However, due to a range of factors that have affected the release of the April 2020 UK Edition of SNOMED CT, the mapping files will not be available until the scheduled SNOMED CT UK Edition Release on 1 April 2020.

    Following a review of options to provide the mapping files earlier, we are preparing an interim workaround which will provide SNOMED CT to OPCS-4.9 maps four weeks prior to implementation. This was presented at the SNOMED CT Implementation Forum on 4 February 2020 with no issues raised.

    Workaround

    By 2 March 2020, we will publish on TRUD an interim 28.1.2 patch release of the SNOMED CT to OPCS-4.9 map (single Snapshot file only). If suppliers apply this patch data on top of the existing 28.0.0 (October 2019), that is remove one Snapshot file and replace it with a later-dated identical one, this will allow affected Trusts to mostly comply with the OPCS-4.9 mandate. Full compliance will be achieved by rolling out 29.0.0 (April 2020) when available.

    There are 223 new four-character codes in OPCS-4.9, all of which can be reached in this 2 March 2020 patch release. Some of the new OPCS-4.9 codes have been added as alternative map targets for existing (October 2019) concepts. However, it is important to note that for the April 2020 release, some of the new OPCS-4.9 codes will be reached via maps attached to new and more appropriate SNOMED CT concepts.

    Note: In our mapping technical and implementation guidance we advise that not all OPCS-4 and ICD-10 codes can be reached from SNOMED CT concepts. We, therefore, recommend that providers of systems and applications which incorporate SNOMED CT include the classification Codes and Titles and Metadata files to facilitate manual input of codes where these are not reached from a SNOMED CT concept.

  • Update to 2019-20 release schedule for SNOMED CT UK Edition maps to ICD-10 and OPCS-4

    Dear colleague,

    The next release of SNOMED CT to ICD-10 and OPCS-4 maps will be on 01 April 2020.

    The development of and migration to a new terminology tooling platform as well as the subsequent deferment in 2019 of taking the SNOMED International releases for January 2019 and July 2019 has resulted in compressed timelines for mapping.

    The impact of this is that prioritisation has been given to new UK SNOMED CT content including the update of existing maps to reflect any changes as a result of the new codes in OPCS-4.9. This means only a minimum number of new international concepts will be mapped to the classifications before the map editing freeze date.

  • Change to the file names within the UK Drug Extension, RF2 release

    Published: February 2019

    Please be advised, to align with SNOMED International requirements there has been file name changes to the latest 26.5.0 UK Drug Extension, RF2 release. The changes are “en-GB” to “en”. This is within the Full, Snapshot and Delta packs.  An example of this change is shown below.
